The benefits of using video review technology in trauma performance improvement (PI) are well documented. Trauma PI leaders have found that video review can help uncover many quality issues that are not apparent in performance data and do not come to light during team debriefs or multidisciplinary meetings.
According to a recent survey of EAST members who practice at Level I and II trauma centers, “62% of users reported that the use of TVR [trauma video review].at their center led to performance improvement initiatives and 41% of users reported it has changed institutional practice guidelines.”
But while most trauma leaders understand the power of video, many centers have not yet implemented a video review strategy. There is a common perception that a range of legal, technical and logistical barriers prevent trauma programs from adopting video review.
RESPOND Review — which is part of the RESPOND Clinical trauma suite — is a proven technology platform that overcomes the barriers to using video review in trauma PI.
The Review system captures trauma resuscitations in high-definition 4K video along with high-quality audio and patient vitals. It allows trauma leaders to observe their teams in action, refine resuscitation processes, educate providers and improve overall performance.
RESPOND Review is based on 20 years of development in trauma centers across the U.S. As a result, the RESPOND team is able to help trauma centers overcome the three typical barriers to trauma video review and achieve the benefits of video-assisted PI.
1. Patient privacy laws and regulations
Trauma program leaders who want to explore video review are often confronted with legal and regulatory concerns. Compliance staff may be uncertain about the privacy and security implications. Hospital counsel may raise concerns about medicolegal issues.
RESPOND Review is a HIPAA-compliant video platform that meets and exceeds all requirements governing patient privacy and provider liability. The system uses the highest available level of encryption to ensure complete data security. And role-based access restricts patient video and records to appropriate staff, enabling full audit logging.
In addition, RESPOND Review can be configured to hospital-specific privacy policies: for example, studies can be locked until patient consent is obtained.
From a medicolegal perspective, RESPOND Clinical auto-deletes video studies at the end of the state’s QI requirement period. This ensures provider safety while giving the trauma team full access to resuscitation videos for PI activities.
2. Coordinating with IT to deploy video review
Working with IT to implement a new technology can be complicated and time-consuming. RESPOND Clinical has worked with IT departments in hospitals of every size and type. In the process, the RESPOND team has encountered and resolved the full range of technological barriers to trauma video review.
- Easy implementation: The RESPOND database runs on Microsoft SQL Server, which enables easy implementation to the hospital’s existing Enterprise SQL system and ensures greater uptime with fewer demands on IT staff.
- Minimal hardware: The RESPOND virtualized server can be stored at the hospital’s data center, which reduces required equipment purchases and the need to introduce new equipment onto the hospital’s IT network.
- Streamlined administration: Authentication through Active Directory allows users to log in with their existing domain accounts, simplifying user management. Users have access to RESPOND’s 24/7 “Always Available Support”, which decreases the support burden on IT.
The RESPOND team begins by creating a full IT plan, laying out exactly what needs to be done. The team also works with IT during on-site installation of equipment. Every installation is served by a dedicated RESPOND project manager, ensuring prompt resolution of any issues that may arise.
3. Incorporating video review into staff workflows and PI processes
Many trauma leaders are wary of adopting new technology. Training staff on any new system can be challenging, and some tech solutions provide only incremental benefits.
The RESPOND system is based on two decades of iterative development with trauma providers, resulting in a system that is easy to use and provides a range of advanced PI tools. The RESPOND Review system:
- Captures video from multiple camera angles and displays patient vitals side-by-side with resuscitation video, providing a comprehensive view of trauma activations.
- Automatically captures 15 minutes of video prior to team activation (via the system’s “rewind” feature), allowing program leaders to review and optimize critical pre-arrival details like room setup, role communication and team leader brief.
- Lets users flag clinical events, allowing PI reviewers to track elapsed time between key events and easily reference critical points in a resuscitation.
- Includes a “live view” mode that allows program leaders (such as the TMD or TPM) to watch resuscitations as they happen and get a real-time view of team performance.
RESPOND Review integrates with Epic, Cerner and Meditech to automate study capture so staff can concentrate on patient care. The RESPOND team is always available to train new staff and make sure they can use the video review system efficiently and effectively.
Part of a complete suite of trauma tools
RESPOND Review is just one module of the RESPOND Trauma Management System. Other modules include RESPOND Vitals (described above) and RESPOND Arrivals, which captures staff arrival times to the trauma bay using the hospital’s existing badge system. The Arrivals system sends timestamp data to the EHR in real time and provides dynamic reports and analytics, serving as a versatile tool in trauma PI.
What’s next? In 2024, RESPOND will introduce RESPOND Registry, a new option for trauma programs looking to build a dynamic trauma data platform. RESPOND Registry is a fully customizable trauma registry software system that integrates with major EHRs and enables real-time data validation.
The module-based RESPOND Trauma Management System can be configured to meet the unique needs of any trauma center. The RESPOND team works with IT, legal, compliance, finance and clinicians to ensure a successful implementation that strengthens trauma PI and leads to better trauma program performance.
