Does your trauma center use video review to assess and improve team performance? Trauma System News recently spoke with Ryan Dumas, MD, about the benefits of trauma video review, how it can support trauma PI and patient safety, and how to successfully implement a trauma video review program.
Dr. Dumas is an associate professor of surgery at UT Southwestern Medical Center and the associate trauma medical director at Parkland Memorial Hospital in Dallas.
[0:35] Trauma video review captures data missed by the EMR
- The difficulty of reconstructing a trauma resuscitation from EMR data
- Video review is very effective for capturing time stamps and time intervals
- More important, it is the only way to assess intangible factors like teamwork and leadership
[2:50] What commitment of staff and time is necessary?
- A trauma video review program is resource-intensive
- For that reason, trauma program leaders need to be thoughtful about goals and processes
[4:59] Uses and benefits of video review in trauma PI
- Program leaders must establish the “audit filters” that will trigger a video review
- Parkland uses video review to identify opportunities for improvement
- Focus is on processes such as pre-arrival time-out, EMS hand-off, pre-OR debrief
- Video review has been incorporated into the center’s weekly peer review conference
- The process helps trauma team members develop “pattern recognition”
- Video review helps trauma teams prepare for high-impact, low-frequency events
[9:35] Essential steps to implementing a trauma video review program
- Get buy-in from everyone who will be “under the lens”
- Get approval from administration and legal (and know that video review is protected)
- Get funding (many centers have had success through grant opportunities)
- Select a video review system (many video review products are now available)
- Establish clinical processes to run a sustainable system
[13:28] Three pitfalls to avoid with trauma video review
- Pitfall #1: Failure to identify and engage stakeholders
- Pitfall #2: Using video review in a punitive manner
- Pitfall #3: Failing to set up program infrastructure (roles, use within peer review and PI, etc.)
[15:00] Using video review to decrease variability in trauma care
- Trauma video review is the perfect tool for research
- It is a key tool for improving patient outcomes by reducing team, provider and system variability
Dr. Dumas will deliver the keynote address on the topic of trauma video review at the 2024 Erlanger Trauma & Surgical Critical Care Symposium.
